Medical Statement

All divers taking PADI courses are required to fill out this form before any diving activity can take place.

The purpose of this Medical Questionnaire is to find out if you should be examined by a doctor before participating in recreational diver training. A “Yes” response to a question does not necessarily disqualify you from diving. It just means that there is a pre-existing condition that may affect your safety while diving and you must seek the advice of a physician prior to engaging in dive activities.

If you answer “Yes” to any of the questions, you must get a doctor to sign off that you are fit to dive and bring that form with you. There are no exceptions to this. It is highly advised that you check with your own doctor in your home country to verify if scuba diving is safe for you.
